How does Seroquel XR work?

Seroquel XR works by blocking the activity of dopamine (a chemical in the brain that controls mood and emotions) in the brain, which may enhance feelings of well-being and happiness in people with schizophrenia.

Dopamine plays a vital role in mood regulation. By blocking its activity, Seroquel XR helps reduce psychosis symptoms, while increasing the amount of serotonin in the brain. Seroquel XR is not approved for use in children younger than 18 years of age.

Primary Outcome

A total of 962 patients with bipolar I disorder, bipolar II disorder, MDD, or other major depressive disorder (MDD) were randomly allocated to Seroquel XR or Seroquel SR (quetiapine) and received either Seroquel SR or placebo (both doses at the beginning of treatment) for a period of 3 weeks. Patients were then evaluated at baseline and at 3 months.

Secondary Outcome Measures

A total of 654 patients with bipolar I disorder, bipolar II disorder, or MDD were randomly assigned to Seroquel XR or Seroquel SR at 3 months, and were then evaluated at baseline and at 3 months.

Change in Diagnostic Measure

The primary endpoint was change in the BDI (Beck Depression Inventory-II) scores over 3 months (baseline to 3 months).

The secondary endpoint was change in the BDI (Beck Depression Inventory-I) scores over 3 months (baseline to 3 months).

Follow-Up

The primary endpoint was change in the MADRS (Beck Depression Rating Scale-Revised) at 3 months.

The secondary endpoint was change in the BDI (Beck Depression Inventory-II) scores at 3 months (baseline to 3 months).

Intervention and Follow-up

The study was stopped early in the period of 3 weeks and continued after baseline, and at 3 months. All participants were required to take the study medication at the same time every day for 3 weeks.

Inclusion criteria were as follows:

  1. Bipolar I disorder (at least 13 years; DSM-V)

  2. Bipolar II disorder (at least 5 years; DSM-V)

  3. Bipolar disorder lasting at least 2 weeks

  4. Bipolar disorder that was co-morbid with a depressive disorder, in which BDD was the co-morbidity

  5. Patients who were clinically stable on a Hamilton Depression Rating Scale (HAM-D-S), at the beginning of the study (baseline to baseline)

  6. Patients who had not yet reached the end of treatment (3-month assessment).

Exclusion criteria were as follows:

  1. Bipolar disorder that was comorbid with major depressive disorder, in which BDD was the co-morbidity

  2. Patients with a history of major depressive disorder (M& D) in addition to a depressive disorder within the previous 6 months

  3. Patients with a history of major depressive disorder and a history of bipolar disorder

  4. Patients who had been taking any type of antidepressant for more than 6 weeks in the previous 6 weeks.

  5. Patients who had a history of major depressive disorder within the previous 3 months (including an MDD diagnosis).

  6. Patients with a history of major depressive disorder and a history of bipolar disorder within the previous 6 months.

The study was stopped early in the period of 3 weeks and continued after baseline and at 3 months.
